Biometrics is required for the applicants for who have not have not given Biometrics after 2nd November 2015
Applicants valid passport is required ( Handwritten passport is not allowed )
Visa form to be filled online - http://www.e-konsulat.gov.pl/ www.mumbai.mfa.gov.pl
Letter of Invitation must be with full details of passenger’s first page of passport and business profile requesting for visa addressed to Consulate General of Poland, Mumbai in addition to all above documents.
One copy of the Invitation has to be faxed from Poland to the consulate and one copy along with the documents to VCS Office.
2 Photographs - 35 x 45 mm, 80% face size, white background
Original Covering letter
Ticket copy
Overseas Medical Policy
Original Bank statement of last 3 months with bank seal & signature on all page & adequate minimum balance
Salary slip of last 3 months with company round seal & signature (not mandatory for director/partner's of the company)
